The immediate and surrounding work area has been reviewed for any uncontrolled risks and controls have been put in place as required. *

If not safe or unsure, do not start. Contact the supervisor.

Persons have the necessary skills, qualifications, training and competencies to perform their tasks. *

The site's emergency procedures are understood? *

If No, refer to the site specific WHS Management Plan in the safety documents.

Persons have access to a first aid kit and a worker trained in first aid? *

If No, contact the supervisor.

The site specific rules are understood? *

The procedures for reporting incidents and injuries are understood? *

The correct PPE is available for persons undertaking their task? *

If No, do not start. Contact the supervisor.

Extension leads, corded power tools and appliances are tested and tagged, guarded, serviced and maintained as applicable? *

If No, do not use. Remove from service. Make good before commencing work.

Safety Data Sheets are available for hazardous chemicals? *

If No, do not use the product / substance. Contact the supervisor.

Any fine silica dust created during work activities will be managed by water suppression, local exhaust ventilation or PPE where the product cannot be eliminated. Others in the immediate work area will be considered. *

If No or uncertain, refer to the product's SDS. Do not use the product / substance. Contact the supervisor.

The location of the site amenities (toilet, water etc.) are known of. *

If No, contact the supervisor.

If working at a facility attended by children (such as a school or early learning centre), I confirm having a current 'Working with Children' check. *

Upon leaving the site, it will be left secure from unauthorised access. Any fence panels removed will be reinstated to a secure condition. *

If any panels are missing or the fencing is incomplete, contact the site supervisor.

I acknowledge that I am not permitted to alter scaffolding unless authorised and appropriately qualified. If scaffold requires altering / is not safe, I will contact the supervisor.

I understand that persons are responsible for cleaning up their own rubbish and leaving the workplace in a safe and secure condition.

I will wear the required PPE at all times (hard hat, hi-vis, safety boots, eye protection).

I will not work under the influence of alcohol or drugs.

I will report all hazards, near misses and incidents to the site supervisor immediately.
